The BMJ · The role of masks and respirators in preventing respiratory infections in healthcare and community settingsThe covid-19 pandemic saw frequent changes and conflicts in mask policies and politicization of masks. On reviewing the evidence, including studies published after the pandemic, the data suggest respirators are more effective than masks in healthcare, but must be continuously worn to be protective. Healthcare and aged care settings amplify outbreaks, so protection of patients and staff is paramount. Most guidelines assume risk is only present during close contact or aerosol generating procedures, but studies show intermittent use of respirators is not protective. New research in aerosol science confirms the risk of infection is widespread in health facilities. In community settings, any mask use is protective during epidemics, especially if used early, when combined with hand hygiene, and if wearers are compliant. Community use of N95 respirators is more protective than surgical masks, which are more protective than cloth masks, but even cloth masks provide some protection. Mask guidelines should be adaptable to the specific context and should account for rising epidemic activity, and whether a pathogen has asymptomatic transmission. The main rationale for universal masking during pandemics is asymptomatic transmission, which means risk of transmission cannot be self-identified. The precautionary principle should be applied during serious emerging infections or pandemics when transmission mode is not fully understood, or vaccines and drugs are not available. If respirators are not available, medical or cloth masks could be used as a last resort. Data exist to support extended use and reuse of masks and respirators during short supply. In summary, extensive evidence generated during the covid-19 pandemic confirms the superiority of respirators and supports the use of masks and respirators in the community during periods of high epidemic activity. Some gaps in research remain, including economic analyses, research in special population groups for whom masking is challenging, and research on countering disinformation.

I have been suffering from #LongCOVID for a year now. It is debilitating. And I keep wearing my #facemask because it is effective in protecting me as a vulnerable person because noone else gives a shit anymore. It's disgusting.

#Pakistan shuts primary schools for a week in #Lahore due to dangerous #AirQuality

LAHORE, Pakistan (AP) — "Dangerously poor air quality on Monday forced Pakistani authorities in the cultural capital of Lahore to close primary schools for a week, government officials said, after the air-quality index hit a record high over the weekend.

"The measures in Lahore were part of a larger effort to protect children from respiratory-related and other diseases in the city of 14 million people. The government said everyone in Lahore was required to wear a #FaceMask.

"Fifty percent of employees must also work from home as part of a '#GreenLockdown' in the city, the government said, adding that barbecuing food without filters was banned and motorized rickshaws restricted. Wedding halls must close at 10 p.m. and artificial rain is likely to be used to combat the pollution.

"The air-quality index in Lahore exceeded 1,000 over the weekend, a record high in Pakistan.

"Toxic gray smog has sickened tens of thousands of people, mainly children and elderly people, since last month when the air quality started worsening in Lahore, the capital of eastern #Punjab province bordering India.

"The government has also banned construction work in certain areas and fined owners of smoke-emitting vehicles. Schools will remain closed for a week because of the pollution, according to a government notification.

"The concentration of PM 2.5, or tiny particulate matter, in the air approached 450, considered hazardous, the Punjab Environment Protection Department said.

"Lahore was once known as a #CityOfGardens, which were ubiquitous during the Mughal era from the 16th to 19th centuries. But rapid #urbanization and surging population growth have left little room for #greenery."

"Gov. Kathy Hochul said Thursday that she is considering a ban on face masks in the #NewYorkCity subway system due to concerns about people shielding their identities while committing antisemitic acts.

"Hochul, a Democrat, told reporters the exact details of the policy are not clear but it would contain 'common-sense exemptions' for health, cultural or religious reasons. Many people concerned about #COVID19 and #AirPollution routinely wear masks on the subway.

"Hochul said she was in talks with lawmakers on potentially crafting a bill.

"At at news conference in Albany, the governor said she was moved to act after 'a group donning masks took over a subway car, scaring riders and chanting things about Hitler and wiping out Jews' on Monday night.

"It was not clear exactly what incident she was referring to, but it could have been a conflation of different episodes related to pro-#Palestinian #demonstrations that day in #UnionSquarePark.
