So what is the current consensus (if any) on choice of fluids for resusc in sepsis. Balanced vs NS? (TBI not considered). SMART, SALT-ED and the NEJM meta analysis towards balanced vs BaSICS' null findings? Any practice changes? @Kidney_boy @hswapnil #askrenal
@draashish @Kidney_boy this is our last review after BASICS http://www.nephjc.com/news/basics
There was PLUS from NZ (also negative) after that
Then a Bayesian MA from the same group which claimed superiority (mostly driven by the Vandy trials which are an outlier to me)
But one could argue RL is not worse - so why not use that? If cost is not a consideration
That’s assuming you think RL = Plasmalyte which is not true (and should be discussed when we discuss BEST FLUIDS) later this month
@hswapnil @Kidney_boy I really hope you are hinting towards a @freelyfiltered episode. . Will wait for the discussion!
@hswapnil @Kidney_boy yeah. Had read that @NephJC review. We had a discussion in our noon conf and couldn't come to a good conclusion either way, but the MA was leaning very much towards balanced.
